False cliffs in completeness charts
A missing load receipt can invent a completeness drop that the pipeline never caused.
Pipeline meetings escalate when a path chart shows a steep cliff between adjacent stages after a release. Sometimes that cliff is real. Often the second stage never fired after a permission change, or only fired in one environment.
Before rewriting roadmaps, walk the path on the schedules your stewards actually run and watch the live markers. Note every silent miss. Ask whether the step is client-confirmed, warehouse-confirmed, or both.
